*To provide glamorous children events and services that brings fun, excitement and decadent spa treatments, Eminence Mobile Massage & Spa Service table massage requires under garments/clothing or bathing suits remain worn and wrapping required for teens, pre teens and for children ages 4-12 .
Eminence Massage LLC_ to provide child/minor/person under my guardianship with therapeutic age appropriate teen, children massage and facials services as deemed appropriate to mobile spa party services. One host/parent guardian/event sponsor over the age of 21 must also be present onsite at location, remain present at event location and give consent for minors to receive Eminence teen/children facial, massage services. If reserved event planner may also take pictures of onsite location to provide the professional advice for all stations to accommodate and equipment, massage and facials.
I understand that I am financially responsible for the minors, and that all statements contained in this consent apply equally to me and the minors. I understand spa services are to fully accommodate children and or teens attending event, adults may not engage in massage and or facials unless organized prior to services.
I have read the above information and give permission for Eminence Massage to cater these services on site at provided locations. I also approve of any future spa party sessions
