I understand that my data will be strictly confidential. This clinic does not sell, share, or resell information.
I confirm that all information in this form is true and accurate.
I confirm that if I hold some important information and complications happened, the clinic will not be liable.
I release this clinic and hold harmless against any claims, expenses, damages, and liabilities.
I understand, have read, and completed this questionnaire truthfully. I agree that this constitutes full disclosure and that it supersedes any previous verbal or written disclosures. I understand that withholding information or providing misinformation may result in contraindications and/or irritation to the skin from treatments received. I am aware that it is my responsibility to inform the esthetician/skin care therapist of my current medical or health conditions and to update this history. The treatments I receive here are voluntary and I release this institution and/or skin care professional from liability and assume total responsibility thereof.
I also hereby grant and authorize Eminence Esthtetician the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures, video, and/or audio taken of me to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, press kits, websites, social networking sites, and other print or digital communications without payment or any further consideration. This authorization extends to all languages, media, formats, and markets now known or later discovered. I waive the right to inspect or approve the finished product wherein my likeness appears, including a written or electronic copy. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I at this moment hold harmless and release Eminence Massage LLC from all liability, petitions, and causes of action that I, my heirs, representatives, executors, or any other persons may make while acting on my behalf or on behalf of my estate.
Cancellation and Rescheduling Policy
ARRIVAL TIME:
Please aim to arrive 10 minutes before your scheduled appointment time. If you arrive after your scheduled appointment time, it may not be possible to extend the time available for your booked service; if your service is shortened due to your late arrival, you may still be charged the full cost of the service.
HEALTH DISCLOSURE:
I certify I have disclosed all relevant medical conditions, medications, allergies, pregnancy status,
injuries, surgeries, implants, and skin conditions that may affect today's treatment.
CONSENT & LIABILTY RELEASE :
I understand facial treatments may include cleansing, exfoliation, extractions, masks, massage,
LED, microdermabrasion, chemical exfoliation, radiofrequency, ultrasound, or similar services.
Possible reactions include redness, peeling, swelling, bruising, sensitivity, or allergic reactions. I
voluntarily consent to treatment and assume the ordinary risks associated with these services. To
the fullest extent permitted by law, I release Eminence Massage LLC and its practitioners from
liability for ordinary accidental injuries or adverse reactions arising from treatment, except where
caused by gross negligence, willful misconduct, or liability that cannot legally be waived.
DEPOSITS:
For all appointments which are not prepaid either by sponsor, a gifted service, or a payment in advance, Eminence requires payments in advanced two months before your scheduled appointment. If it has not been paid, Eminence Massages reserves the right to cancel your appointment.
CHANGING YOUR APPOINTMENT:
24 hours' notice is required to reschedule or cancel a booked appointment, if done later than that Eminence Massage reserves the right to charge the full price of the service or redeem the under your name, if applicable.
